For optometrists to request an official score report to be released to a state board or for personal records.
Order Certificate
For optometrists to order an additional or replacement exam completion certificate. (Candidates who pass all exam parts – Parts I, II, III, and TMOD – will automatically be issued a free certificate to recognize this achievement. Certificates will be mailed within 12 weeks of exam passage and graduation.)
